The following information will assist you in the admissions process for CHPA:

You may fill out an application on the Admissions Tab of this website or come into our Admissions office located at 2500 W. 18th Street. You need to access our office through the 18th Street parking lot entrance. We are located in Building 10, Room 20, directly behind the gray modular building.   Once you have filled out the application, you will receive a phone call from our Admissions and receive instructions on any documents needed as well as spots available within each grade level applying for.
  • Step 2: When enrolling, please bring in or email/fax proper documents such as birth certificate, updated immunization record, and the student’s last report card and/or academic transcript.
  • Step 3: Once all paperwork has been completed and all required documents have been received, your child’s enrollment will be finalized. If there are no available spots in your child’s grade level, your child will then be placed on a waiting list. If there is a wait list at semester for the following school year, a lottery will take place in the spring and parents will be notified.
If you are currently have a CHPA student, your child will automatically roll over into the next grade and appropriate school (CCA, ECMS, DHPH).
